ABNORMAL BLEEDING BETWEEN MY MENSTRUAL PERIODS

Hello Doctor,

i have recently seen my gyno because of light bleeding that has lasted for over one month, i have not had a normal menstrual cycle for over a year, some months i have not had a menstrual cycle. The doctor explained to me that iam anovulatory, and prescribed tranexamic acid (500mg) a three day dosage to stop my light bleeding, i was to take three tablets by mouth three times a day i started that 10 days ago,i noticed that the bleeding did slow down, but has not completly stopped, i still have very light bleeding, i have noticed a few very tiny blood clots, i do not need to wear a pad. i have no pain whatsoever.iam on birth control pills for one month currently on my second week to regulate my menstrual cycle then i will be put on clomid for 6 months because i want to become pregnant. i had a ultrasound done on my ovaries in feb which was normal, and 3 weeks ago had blood tests done for thyroid, CBC, etc which came back normal,the gyno also did a pelvic exam and noted nothing was abnormal,in the last 3 months i have dropped 32 lbs to be healthier, and have been exercising more, sometimes after physical exercise the light bleeding will increase should i be concerned that the bleeding has not stopped or does it take time for the tranexamic acid to work?should i see the gyno again? is the bleeding continuing because my hormone's are trying to return to normal?? if you need anymore information do not hesitate to ask please, thankyou.

The tranexamic acid should have started to work in 2 - 3 days. Since you are still bleeding in spite of the tranexamic acid as well as the birth control pills, you need to consult your doctor at the earliest. It is likely that you will need to take high dose progesterone, rather than birth control pills to decrease the bleeding.

Secondary amenorrhea like yours do occur after weight loss due to exercise or heavy dieting, but the clomid should help stimulate the ovaries to ovulate.

But now, you need to inform your obgyn that the bleeding has not stopped.

Why do i need to take high dose progesterone rather than the birth control pills?

BC pills contain both estrogen and progesterone - estrogen causes the growth of the inner uterine lining (endometrium)and you may have further breakthrough bleeding. Progesterone converts the endometrial cells from a proliferative type to a secretory type, preventing further growth.

So, High dose progesterone-only pills are more effective in controlling bleeding.
